Operation Support Centers

Integrated Care Network (ICN)

We work closely with partner companies to offer enhanced care management, education and outreach services to Medicaid members in Alabama who need Long-Term Services and Support (LTSS). The ICN oversees thirteen Area Agencies on Aging (AAA) to make sure our members are receiving the care they need.

Commonwealth Coordinated Care Plus (CCC+)

Shared Health provides services to the CCC+ program in Virginia that focuses on providing LTSS members with managed care.

Medallion 4.0

Additional services are provided to the Medallion 4.0 plan that covers the Temporary Assistance for Needy Families (TANF) and the Children's Health Insurance Program (CHIP) populations. Pregnant women, low income young families and youth in foster care or transitioning out of the child welfare system are all a part of the covered populations.

Diamond State Health Plan

We work with members of the Diamond State Health Plan (DSHP) specific to Long-Term Services and Supports (LTSS). Members of this program live at home, in community based settings or in nursing facilities.
